If your practice is already set up on the Provider Portal, please reach out to providerportalsupport@hpmg.com to get a new user added. The authorized site administrator for your practice will be given access to our new provider portal and instructions to add new users.
If your practice is new to the Provider Portal, please download the Provider Portal Access Request form linked below and email it to providerportalsupport@hpmg.com. A member of our team will reach out to help get your account set up.
If you are experiencing difficulty logging in, email us at ProviderPortalSupport@hpmg.com or call Practice IT Support at 1-800-280-0083. Please be sure to include your user name in the email.
